Overshadowed by the Uniting to Combat NTDs announcement made last week in London (see my post of 2/1/12 and Combat NTDs) was the release of a report on how private sector companies and grant-supported organizations have addressed and can address the need for medicines in bottom-of-the-pyramid (BoP) markets.  “Bringing Medicines to Low Income Markets:  A guide to creating inclusive business models for pharmaceutical companies” (Medicines for BoP) was sponsored by the German Federal Ministry of Economic Cooperation and Development (BMZ) and Sanofi and written by Endeva, a consulting firm offering “enterprise solutions for development.”  While I commend the authors for addressing the opportunities and challenges of the BoP medicines market, I found that they fell short on three points:  not emphasizing the role of a major player, governments; not providing compelling examples with details; and the depth and utility of the guidance provided.

The report starts well with a succinct description of the market’s opportunities and challenges.  Here’s my summary of the salient points made:

  • the demand (need) is large with 1.7 billion people lacking access to the basic, essential medicines and another 2.3 billion with limited access;
  • this 4 billion have annual incomes of under $3000 but spend about $160 billion on health care of which about one-third is on pharmaceuticals;
  • drugs are 2-6 times more expensive in the under-served markets than in developed markets due to taxes/tariffs, middleman mark ups, and supply (inventory) problems;
  • in lower-income countries communicable (infectious) and non communicable diseases or NCDs (e.g., diabetes, heart disease, cancer) contribute equally to a country’s mortality rate while in middle income countries the NCDs contribute more than twice as much as the communicable diseases; and
  • companies seeking to enter the BoP markets are challenged by missing “market enablers” such as the lack of doctors and other trained health practitioners, regulations (in many countries 20-30% of the drugs sold are counterfeit), infrastructure for delivery and stocking of products, financing for businesses providing health care, and insurance plans (70% of low-income patient drug purchasing is out-of-pocket).

The next section of the report provides a “tool,” really a framework description of what everyone in business should know:  to sell a product the customer has to recognize its value, it needs to be in a form she/he can use, and be affordable and available.  Also described are the key actors:  governments, international organizations (like WHO), bilateral agencies (like the BMZ and USAID), foundations, microfinancing organizations and insurers, NGOs, and the private sector.  Unfortunately, the rest of the report is heavily weighted in its examples and recommendations toward the existing dominant actors of the bilateral organizations, NGOs, and foundations.  For example, the report does not mention ways in which the governments, to my mind the key responsible party for providing the right to a basic level of health care, can improve their performance.  One route which I have posted on in the past (“A Plan for Essential Care” 1/20/11 ) is the use of a “health sector wide approach” (or health SWAp) in which the public health system is strengthened by requiring that all donors’ funds are administered by the responsible government agency with full accounting and transparency so that the funded activities, which are typically aimed at specific diseases like HIV/AIDS, polio, and malaria, are coordinated with government efforts to provide basic health services like pre- and ante-natal care and immunizations.  Businesses are most successful over the long term in environments where the risks are known and predictable and governments by proving regulations, infrastructure, and a justice system provide that environment.

I was also disappointed in the report’s case studies (really examples, not studies which would be more in depth).  While the authors note that the more than 100 documented examples from pharmaceutical companies as well as other organisations such as NGOs were analyzed, relatively few provide the promised “inclusive business models for pharmaceutical companies.”  Of the 18 given, four involve solely NGOs and are therefor are not self-sustaining (Cases 5, 13, 14, and 17), and six are marginally relevant to selling into the BoP markets (Cases 2, 3, 6, 7, 8, and 18).   My summary of the cases and comments are as follows:

The report’s final chapter is said to provide “practical guidance to pharmaceutical companies on how to get started with inclusive business, including tips for strategy, internal organisation and partnerships.”  Really it provides general guidance and needed to reflect a better understanding of the pharma industry and its progress in entering emerging markets (I note that, of the organizations proving expert advice used, only 10 of 34 were for-profits).  What and where are certain products needed?  What is the relative level of government support of health care infrastructure in various countries?  What is needed for product approval and registration?  Which local companies may make good partners?  Overall, I give the report an “A” for effort in tackling the BoP biz challenge but a “B minus” for utility.  My amateur opinion is that NGOs, foundations, and bilateral and international organizations should work through governments to build out the country’s health care infrastructure, and the private sector should develop appropriate and affordable products, provide commercial and patient financing, and build supply and distribution chains.

The final selection is from a brand-new album titled Honoring the Local Gods of a concert at Local 269 in New York City on November 7, 2009 with a group called Hollow Bones, including Elliott Levin & Ras Moshe on tenors, Anders Nilsson on guitar, Alex Obert in bass and Michael Wimberly at the drums.

2. Releasing a Fragrance – Wonderstruck

Tay ventured out this year into beauty products (with help from Elizabeth Arden) to create her very own perfume! The scent’s name is a reference to her song “Enchanted” from Speak Now. The fragrance hit stores in October and has gained tons of praise for its yummy scent.

3. Being Crowned Artist of the Year at the American Music Awards

Taylor’s been winning so many awards this year, but this one was pretty big – especially considering Tay was up against some stiff competition (like Adele, Lady Gaga and Katy Perry!). Katy even sent Tay a congratulatory tweet (pretty awesome, huh?)!

4. Becoming a Mommy to an Adorbs Kitty, Meredith

Not too long ago Tay introduced fans to her kitten, Meredith, via Twitter. We still can’t get over how cute she is. Our jaws kind of hurt from all of the “Awws!”

We know Taylor’s been on the receiving end of some pretty spectacular awards, but this award is seriously quite the honor. The most populated city in east Africa, and one of the fastest growing, Nairobi, Kenya has become an extremely strategic regional center for business, banking, development, and politics. A destination hosting a diverse mingling of foreign inhabitants, from emissaries, ambassadors and development agencies to mobile innovators, technologists and consultants, Nairobi has just recently to crept into the international market as a city to keep an eye on.

State owned miner Singareni Collieries Company will finalise the balance of the plant for its 2X 600 MW coal fired thermal power station coming up in Adilabad by March this year. Mr S Narsing Rao CMD of SCCL said that “We will finalise the BOP by March. The process relating to it is already on.” The project coming up in Jaipur mandal of Adilabad district is the first power venture for SCCL. The project is estimated to be INR 5,685 crore. About 70% of it (INR 3,980 crore) will come from Power Finance Corporation and about INR 350 crore has already been extended to SCCL, which will get the remaining 30 per cent of the project funding from its internal accruals and some debt. The Balance of Plant is expected to cost around INR 1,000 crore. The civil works are already on. SCCL has already placed a INR 4,071 crore equipment supply order with Bharat Heavy Electricals Limited for design, engineering, manufacture, supply, erection, testing and commissioning of steam turbines, generators, boilers and associated civil work of the projects. According to Mr Rao, the Phase I of the power plant is expected to be ready by end of 2014 while the second unit would be commissioned in three to six months after that. The project would need about 13,500 tonne coal per day and this would be supplied from SCCL Srirampur and RK mines in Adilabad district. The mines, which were abandoned earlier, still had reserves 160 to 170 million and would last for about 40 years. (Sourced from mydigitalfc.com)

There are a couple of really disturbing shots of Pauly au Naturale. He sort of looks like a caramel-dipped Joyce de Witt. The boys go out to birthday dinner, and the girls plan the surprise party by vetting strippers, and it’s the only time in Jersey Shore history that the words “eminently qualified” ever spring to mind.But, cue sad music, because Mike is upset that he’s not getting his fair share of the birthday attention and storms off to passive aggressively nap in the middle of the restaurant. Here’s a tip, Mike: If you want people to enjoy your company, maybe don’t leave in the middle of dinner in a restaurant to nap? In the restaurant? Ronnie does the logical thing and pipes frosting all over him. Alas, you cannot be cake and eat it too. The meatballs have decided to make a pastry effigy exclusively for Pauly, which, as he had one back at the restaurant, is an embarrassment of dessert riches. There’s no Mike cake, probably because Carvel was out of both Fudgie the Unself-Aware Whale and Herpepuss. Mike mopes his way through the rest of the episode, doing a kind of sad capoeira on the patio. Having decided to be a villain and act villainous throughout the entire episode, he is suddenly upset that everybody sees him as a villain. Don’t worry, Mike, you have the rest of your life to be shockingly ineffective. He ambles off into the sunset, and cue morose post-grunge.”Dropping like flies,” observes Snook. Flies, indeed!Is it possible that we’ll be short a Mike AND a Vinny? Or is this just another gambit for attention? As awful as Mike is, I really don’t want to see him on a bus to Florida, yammering on about getting a real job only to realize that Unit has died next to him. The Year in Music – 1982

The man with the beret, Dave Bickler, walks the mean streets to the big drum beat. Welcome to music in 1982 as Bickler’s band, Survivor, delivered the year’s biggest hit, Eye of the Tiger, which still features in boxing gyms and at the starting gates of sporting events everywhere. Buoyed by the movie in which it featured, Rocky III, Eye of the Tiger spent six weeks at number one.

Also spending six weeks at the top of the charts was a local performance, What About Me? by Moving Pictures. It is now used in a TV commercial for insurance. Toni Basil produced one of the biggest one-hit wonders of all-time, changing the name on an old Racey song and making it all about Mickey (with a few cheerleaders to help her along). Soft Cell sang about Tainted Love, Charlene told us I’ve Never Been to Me and the Dexy Midnight Runners said Come on Eileen.

Bruce Springsteen ditched the E-Street Band to do a very quiet and despondent Nebraska. Billy Joel put up The Nylon Curtain which featured chart hits Allentown and Pressure plus a Vietnam War lament, Goodnight Saigon. Kate Bush confused many with her album, The Dreaming, which featured Rolf Harris on didgeridoo while a man with whom she would create one of the best duets of all time, Peter Gabriel, told us to Shock the Monkey on his fourth self-titled album.

Elsewhere, Icehouse gave us Primitive Man which included Great Southern Land, The Church said they were Almost With You, the members of Split Enz spent Six Months in a Leaky Boat while Men at Work were Down Under. The year would end with a little album by a former child star. Michael Jackson’s Thriller was released on November 30. It would go on to become the biggest selling album of all time at 125 million copies and counting. A young woman had earlier released her first single. While Everybody is not remembered now, it was the start of Madonna.

The Year in Music – 1983

It was the year of Michael Jackson. Singles fell off Thriller, starting with Billie Jean and including the title track, Beat It and Wanna be Startin Something. Everyone who could moon walk did, and lots of others tried.

But it was also a year of other prominent releases.

Midnight Oil gave us 10,9,8 etc which included Peter Garrett, who was not a member of the ALP, singing about US Forces and the Power and the Passion. Annie Lennox and Dave Stewart released their breakthrough album as Eurythmics. The Sweet Dream album featured the striking self-titled single, the success of which prompted the re-release of Love is a Stranger. That would cause controversy in the US where the androgynous Lennox was thought by some to be a transvestite, upsetting American sensibilities. By November of 1983 they had released their third album, Touch, which included the torch song Who’s That Girl?

U2 had its first commercial success with the album War while at the other end of the career spectrum, The Clash – at the time one of the biggest bands in the world – sacked Mick Jones.

Unfortunately, we saw the faces of the members of Kiss without make-up for the first time – and it wasn’t pretty. Cutting edge teenagers and uni students were delivered the Violent Femmes first album, Talking Heads was Speaking in Tongues, The Police had Synchronicity, Billy Joel said he was an Innocent Man, Madonna gave us her self-titled debut album while The Cure played with their Lovecats on the Japanese Whispers album.

While we tried to dance like Peter Garrett, Australians listened to a stand-up comic – Austen Tayshus had more puns than you could poke a stick at in his Australiana effort. Pat Wilson said she was a Bop Girl, Goanna chartered with Spirit of the Place, Australian Crawl urged us not to be Reckless and Redgum lamented I was Only 19.

The Year in Film – 1982

It was a year that we fell in love with an extra terrestrial, a man in drag and a re-born Spock. The biggest box office smash of the 1980s, E.T: The Extra Terrestrial, had everyone headed to the cinema for a dose of escapism that also featured a very young Drew Barrymore. It would remain the biggest grossing film until a few dinosaurs in Jurassic Box maurauded their way through the 1990s.

Dustin Hoffman won plaudits and fans for his portrayal of a man forced to dress as a woman to get work on TV in Tootsie. The Star Trek franchise gave us a second film, The Wrath of Khan, which saw Spock killed off (only to resurface in the next film). The sprawling film Gandhi was also released. It would go on to win eight Oscars including best director Richard Attenborough and best actor for Ben Kingsley.

Other big films released through the year included Annie, 48 Hours (featuring Eddie Murphy), Porky’s, Poltergeist and also one now recognised as an all-time classic, Blade Runner. It was also another good year in Australian film with The Man from Snowy River, featuring a young Sigrid Thornton and a much older Kirk Douglas (in two roles) released. It remains among the top 10 Australian films by takings with its mix of myth, horses and whips. The Year of Living Dangerously, starring Mel Gibson, garnered an Oscar for Linda Hunt who played a role as a male dwarf.

One of the sadder notes of the year in film was that Grease 2 was released. Nothing like its predecessor, its only gift to film was the casting of Michelle Pfeiffer in her first major role.

The Year in Film – 1983

It was the third film but became the sixth, featured a bunch of furry bears and is still being re-touched by its maker. Return of the Jedi was released in May 1983, ending the Star Wars trilogy before George Lucas turned it into a six part epic. It was the biggest film of the year, left many Star Wars fans satisfied and playing with their Star Wars merchandise. It is also the film that featured that bikini worn by Princess Leia which had a starring turn in an episode of Friends many years later.

Apart from Luke, Leia and Han, film in 1983 included another major turning point – the advent of soundtrack dependent movies. The soundtrack to Flashdance famously did not sell through Monday to Thursday, but had kids lining up for copies on days they went to the pictures and saw the film about a woman called Alex who was a welder who wanted to be a serious dancer. Staying Alive, featuring a dancing John Travolta (whose best dancing would come in Pulp Fiction), was also released as were notable efforts Terms of Endearment, a little Tom Cruise movie titled Risky Business, Mr Mom, another film with a 1960s heavy soundtrack, The Big Chill, a dog-lover’s worst nightmare, Cujo, and the classic Scarface.

Keeping up our love affair with horses, the Australian film Phar Lap was released, bringing in a near $10 million through the box office. Something that would today be labelled a chick-flick, Careful, He Might Hear You, had people reaching for their tissues over their Jaffas in the nation’s cinemas.

Australian Television 1982 and 1983

We started 1982 with Channel Seven introducing Sons and Daughters – including Pat the Rat, the Hamiltons and the Palmers – to our screens for what would be a six season run and nearly 1000 episodes.

Over at Channel Nine we got introduced to a new breakfast show called National News Today. Its title would be shortened after a few weeks to Today, and remains on our screens.

Kingswood Country was our favourite comedy show, we tuned in every night to see Tony Barber on Sale of the Century, Prisoner provided plenty of toughness while we were just about to get used to Jana Wendt on 60 Minutes.

We also liked our mini-series, with A Town Like Alice of particular interest. Over in the US, David Letterman started his late night variety show while other notable debuts included Remington Steele, Knight Rider, Cheers, Family Ties and Cagney and Lacey. They said goodbye to Mork and Mindy and The Incredible Hulk.

The following year we watched the series 1915, stuck by A Country Practice while the kids watched a show fronted by the former lead singer of Skyhooks.

There were some significant TV endings in 1983. We farewelled the Sullivans after seven years, while The Young Doctors performed their last operations just a few weeks later. By year’s end it was time for the Lanky Yank, Don Lane, to sign off his show.

The biggest event of the year was the end of MASH which gave way to the short-lived series, After MASH. The finale of MASH would become the most watched show on American television.

Also ending that year were Little House on the Prairie, The Greatest American Hero and Laverne and Shirley. Starting that year included The A Team, Webster, Fraggle Rock and Scarecrow and Mrs King.

California Governor Jerry Brown has signed a groundbreaking bill that requires public schools to include the historical contributions of LGBT individuals in their social studies lessons. The law — entitled the Fair, Accurate, Inclusive, and Respectful (FAIR) Education Act — could begin taking effect as soon as the 2013-2014 school year.

California will be the first state in the nation to require schools to revise textbooks and procure teaching materials that cover the contributions and roles of sexual minorities throughout history.
